Hello everyone! I hope someone can help me!!!
I have a three year old who will be moving up to my class soon. I believe she is 3 and 1/2. I do not know the extent of her disabilities or the reason/cause or even if the parents know. She is unable to walk unsupported, she wears braces in her shoes, sometimes has hearing aids on, cannot communicate but sometimes uses handmotions but we don’t know what they mean, if anything. She has attended the school for some time now and as far as I know, no one really knows exactly how to take care of her. Aparently an individual care/edu plan was never established.
Where can I find info. about making an individual care plan for a 3 year old? Is there a basic form I can at least get started with?
Thanks!!!!!!!!!!

If she was on her way to my room, I would look at her file, medical records, any interventions or services that she may have already had. If she does not have anything, determine if she needs a O.T., P.T. evaluation, speech and language and yes, assistive technology. If you do not have staff available, look for a ‘team’ somewhere. In our state there are several large hospitals and clinics that will do complete developmental evaluations. I have a classroom of four basically non-verbal three-year-olds. I am learning to sign really fast! LOL.
Good luck!
It doesn’t sound as though you have enough information considering that you say she has been attending the school “for some time now.” Why is so much unknown?
In any case, you should immediately call in your assistive technology team to assess augmentative communication strategies so that this child can begin to work on communication. How frustrating it must be for her!
