My daughter’s main problem is shifting attention. She doesn’t even hear her teacher call for a transition. She’s considered “boarderline ADHD”. Are there any tools to help her do one thing and be aware of someone else trying to give her new instructions. Do medications help at all with this. She doesn’t have too much trouble maintaining attention.

I will talk to the teacher about your suggestions and hopefully that will do the trick. The Pediatrician won’t consider tryig meds at this point.
Would the teacher be willing to help? Maybe he/she could somehow signal somehow. Maybe walk past the desk and tap it lightly. Maybe another student can help? My daughter had this problem also last year. They had a silent reading period and then science started right after that in the same room. The teacher would come in and start the class and my daughter had no idea because she would be so involved in her book!
