transitional programs

I am looking for transitional programs for kids with learning disabilities who want to go to college but need help in learning to live on their own. Ideally they could live in a residential setting while going to junior college or a vacational program. Thanks!

Some of these programs include the possibility of attending the local community college while in the program, but others are focused only on the independent living, social and vocational skills. Of course, if the student is interested and able college might be an option later after achieving the independent living skills and a little more maturity.
