I’m a learning specialist working with students privately. I’m looking for good questionnaires to give to parents when they start their kids with me so that I can quickly get a better picture of the students’ learning styles and strengths in multiple intelligences (Howard Gardner). I have a quickie on learning style (visual/auditory/kinesthetic) that has only 10 questions and is not that helpful so far. I have a nice questionnaire on MI but it is geared for adults (It’s from our local community college - here’s the link in case anyone’s interested: http://www.metamath.com/lsweb/dvclearn.htm). I’d like something geared toward kids, too.
Any other suggestions for how to quickly get more info on how to best teach an individual? I always learn a lot about how students learn just by working with them over time, but I’d like to speed up the process.

Google the term “visual-spatial learner”. That should give you some good links to help with that learning style, anyhow.
You can read a paper I have written about teaching reading to that style of learner at http://www.learntoreadnow.blogspot.com.
