self-esteem index survey

Hello-
I am wondering if any teachers or parents have a copy of the Self-Esteem Index, a survey of 40 questions, takes about 30 minutes to administer, and has four levels of measurement that deals with self-esteem with family, school, self, and something else that is eluding my thoughts right now. When I look it up on-line it just tells me about it— I want the actual survey. If you have it or if your student or child has taken it, could you please let me know about it?

You would probably have the most luck by going to your school psychologist and asking to see the protocol. I believe that there are a couple of different tests that could be admistered and you would want to see the one they choose to give at your school. You are entitled to see it (the test questions), in the presence of the school, but I do believe that they cannot give you a copy of the test, other than your child’s results.
