Wordsmart CD’s (for the computer) are excellent. Every word is pronounced and used in a sentence. Plus, each word has a very comprehensive spoken dictionary definition that is easily accessed.
Biggest downside is that they are expensive — about $80 each. Also, the company is very aggressive in its selling.
These CD’s sometimes appear on eBay and seem to sell individually for about half-price. Sometimes there is a whole set up for sale (Volumes A-J, I think), and those tend to go for a lot less — really a good bargain if you have the money. Just be sure to get the installation code that comes with the CD’s, if you buy them used.

A good non-computerized program is “Vocabulary for Achievement” from http://www.greatsource.com . A nice feature of this program is that each chapter’s words focus on a theme. This is a workbook approach.
Personally, I prefer the WordSmart CD’s, especially for dyslexics.
